PAYMENT POLICIES
- The fee for service is $100 per 50-minute session. You are responsible for payment, which is to be made at the time of your visit.
- I would appreciate your having your check ready before the session begins so that time during the session is focused on your treatment.
- Appointments will be scheduled directly with me.
- You will be charged my full fee for sessions canceled with less than 24 hours notice. You may cancel by leaving a message on my voice-mail number, (206) 842-5871, which will time-stamp your message.
- A full fee of $100 will be charged if you miss an appointment and do not cancel. Likewise, should I have to cancel your session without 24 hours notice, your next session will be free of charge.
I welcome any questions you may have about these policies.
Because of concerns about client confidentiality and professional independence, I have elected not to participate in any preferred provider insurance panels. Therefore, I will not bill your insurance company for you. I am willing to prepare a statement of services for you to submit to your insurer. You are responsible to determine whether your insurance company will accept and reimburse under such an arrangement, and to provide me with the desired form.
